New Patient Checklist

For new patient appointments, please arrive 30 minutes prior to your appointment time to complete the registration process.

 

IMG_0306Be sure to bring the following items to your first visit:

  1. Insurance card or sliding scale application. All patients who do not have insurance will have their fees determined by our sliding fee scale. To learn more about the sliding fee scale and our payment policies, please click here. 
  2. If you do not have insurance, please be prepared to pay a $15 co-pay for medical visits and a $25 co-pay for dental visits
  3. List of current medications
  4. If possible, please arrive with your new patient forms completed. To access these forms online, click here.

 

 

Squirrel Hill Health Center offers care for children of all ages, but everyone under 18 must have the consent of a parent or guardian.
